COURSE OVERVIEW:

Welcome to the course, Provide Support Services for Older People. In the dynamic landscape of aged care services in Australia, the provision of support for older individuals requires a holistic and person-centred approach that prioritises their needs, goals, and preferences. This program sets the stage on providing services for older people, encompassing a range of essential elements, legal and ethical considerations, and organisational policies and procedures.

In section A, you will gain knowledge of various aspects related to elder care. You will explore the different manifestations and presentations of common health problems associated with ageing, along with the appropriate actions to respond to these issues and when to refer to health professionals. The curriculum covers the aspects of elder abuse, ensuring you can identify and respond appropriately to financial, physical, or emotional abuse or neglect.

You will learn about the roles and functions of health professionals and services involved in elder care, as well as support services available for families and carers. Principles and practices of case management will be highlighted, along with mechanisms to seek feedback from older people and others regarding the effectiveness of individualised plans. The scope and breadth of assistive technologies used across various life domains, and their role in supporting life activities, will also be examined.

Additionally, you will learn the legal and ethical considerations essential for working with older people, along with the organisational policies and procedures that guide professional practice.

In Section B, you will develop skills to effectively establish and prioritise the needs, goals, and preferences of older individuals as outlined in their individualised plans. You will learn to provide services and support activities in consultation with the person, their family, carer, or others identified by the person, ensuring adherence to legal and ethical considerations and organisational policies and procedures.

Supporting the use of assistive technologies to meet individual needs will be a key focus. You will learn to outline and clarify all service providers' understanding of the individualised plan and their roles and responsibilities. Recognising signs of financial, physical, or emotional abuse or neglect and reporting these according to organisational policies will be an essential part of your training.

You will be trained to support older individuals in accessing and negotiating resources for delivering identified services and facilitating access to community support agencies to achieve established goals. Recognising when a service or support worker is unable to provide the required level of service and taking action to minimise disruption will be covered. You will also learn to address the impact of support issues on families and carers, providing support and respite as needed.

Communication skills will be enhanced to explain feedback mechanisms on the effectiveness of individualised plans to older people, their families, carers, and service providers. You will seek and report feedback from these stakeholders to supervising health professionals, supporting the person to seek advice and assistance when their goals are not being reached. Identifying gaps in assistive technology needs and obtaining feedback on the effectiveness of individualised plans from service providers will be part of your practical training.

Finally, you will provide services according to an individualised plan for at least three older people requiring different levels or types of support, including provision of support to their families and carers. You will seek and report on the effectiveness of service provision for each individual.

This course equips service providers with the knowledge, skills, and ethical frameworks necessary to deliver high-quality, person-centred care to older people in the Australian context. Through a commitment to dignity, respect, and collaboration, service providers can support older individuals in living fulfilling and meaningful lives as valued members of their communities.

This course is designed to align with the content outlined in the Nationally Recognised Unit of Competency CHCAGE009.

LEARNING OUTCOMES:

Section B: Performance Tasks

·       Establish and prioritise the needs, goals and preferences of the older person outlined in their individualised plan.

·       Provide services and support activities in consultation with the person, their family, carer or others identified by the person in accordance with legal and ethical considerations and organisational policies and procedures.

·       Support the person’s use of assistive technologies in meeting their individual needs.

·       Outline and clarify all service providers’ understanding of the individualised plan and their roles and responsibilities.

·       Recognise signs consistent with financial, physical or emotional abuse or neglect of the older person and report according to organisational policies and procedures.

·       Support the older person to access and negotiate resources to deliver identified services.

·       Support the person to access community support agencies to facilitate the achievement of established goals.

·       Recognise when a service or support worker is no longer able to provide the level of service required and take action to minimise disruption to service delivery.

·       Recognise impact of the older person’s support issues on the family, carer or others identified by the person and refer according to organisational policies and procedures.

·       Provide support and respite for family, carer or others identified by the person.

·       Explain to the older person, their family, carer, others identified by the person, or service providers the mechanisms for providing feedback on the effectiveness of the individualised plan.

·       Seek feedback from the person and their family, carer or others identified by the person and report to supervising health professional.

·       Support the person to seek advice and assistance from relevant health professionals when their goals are not being reached.

·       Consult with the person to determine gaps in assistive technology needs and report according to organisational policies and procedures.

·       Obtain feedback from service providers on the effectiveness of the individualised plan and report to supervising health professional.

·       Provide services according to an individualised plan for at least three older people requiring different levels or types of support, including provision of support to their family, carer or others identified by the person

·       Seek feedback and report on effectiveness of service provision to each of the above older people.
