Welcome to the Coordinate Services for People with Complex Needs in Case Management course. This course only includes a challenge assessment designed to assess your knowledge and skills required to demonstrate competence in establishing service coordination functions, working with people to identify service needs, coordinating multiple providers, supporting access to services, managing communication requirements, monitoring progress and completing case documentation according to case management frameworks, consent requirements, duty of care responsibilities and organisational procedures.

Coordinating services for people with complex needs in case management involves working with the person to agree on service provision requirements, identifying suitable services and expected outcomes, obtaining consent to exchange information, negotiating collaborative arrangements with providers and supporting access to multiple services. This work is important because people with complex needs may require coordinated support across different services, clear communication preferences, interpreter or translation assistance, prioritised needs, reduced service duplication and ongoing monitoring to ensure changing needs, barriers and progress toward goals are addressed appropriately.

The course assessment focuses on your ability to establish service coordination within a case management framework; work with the person to determine and agree service requirements; develop plans identifying services, availability and outcomes; obtain consent to identify and exchange information; negotiate collaborative arrangements with providers; support access to multiple services; identify and maintain duty of care; explain the service coordination role; establish communication requirements and preferences; assess and arrange interpreter or translation support; identify barriers with the person and services; prioritise needs and communicate them to providers; facilitate meetings to coordinate roles and responsibilities; monitor goal progress, feedback and service duplication; and identify further support for changing needs and complete case documentation.

By successfully completing this course assessment, you will have demonstrated that you can coordinate services for people with complex needs in case management by establishing coordination processes, confirming service needs and consent, working collaboratively with providers, supporting communication access, managing duty of care, prioritising needs, coordinating roles, monitoring progress and completing case documentation according to organisational procedures.

TOPICS COVERED IN THIS ASSESSMENT:

This course assessment is based on the following topics:

1.    Establishing the service coordination function within a case management framework.

2.    Working with the person to determine and agree on service provision requirements.

3.    Developing a plan to identify appropriate services, service availability and expected outcomes.

4.    Obtaining consent to identify and exchange information with other services.

5.    Negotiating collaborative working arrangements and agreed services with relevant providers.

6.    Supporting the person to access multiple services according to identified complex needs.

7.    Identifying, implementing and maintaining duty of care responsibilities.

8.    Providing information to the person about the service coordination role.

9.    Establishing the person’s communication requirements and preferences.

10. Assessing interpreter and translation support needs and arranging access when required.

11. Identifying barriers to outcomes with the person and other services.

12. Prioritising the person’s needs and communicating these with service providers.

13. Facilitating meetings to coordinate service responsibilities and roles.

14. Monitoring progress toward the person’s goals, obtaining feedback and managing service duplication.

15. Identifying further support for changing needs and completing case documentation according to organisational procedures.

However there were just one too many convoluted plot points, sentences just did not flow nicely at times, and I was left with a lot of confusion on my end.
