What is a convex skin barrier and who needs one?

A convex barrier has a curved (inward-facing) profile that applies gentle outward pressure on the peristomal skin, pushing the tissue down and the stoma outward to create a better seal. It is clinically indicated for patients with a flush, retracted, or deeply set stoma; peristomal hernias or skin folds that disrupt the seal of a flat barrier; or persistent leakage despite proper flat barrier use. Convexity should not be used on a stoma with a prolapse or active peristomal irritation without WOC nurse guidance.

What is Flextend and how is it different from a standard extended wear barrier?

Flextend is Hollister's proprietary synthetic skin barrier material formulated for maximum resistance to erosion from liquid output — particularly ileostomy effluent, which is highly enzymatic and corrosive to standard barriers. It is designed to outlast both standard wear and conventional extended wear barriers in high-output situations. It is not recommended for use with skin prep wipes, which chemically interact with the Flextend material and shorten wear time.

What is a floating flange and why does it matter?

A floating flange is an integrated flange that is not rigidly fixed flat against the wafer — it has slight flex and lift that allows the user to place fingers underneath it during pouch coupling. This significantly reduces the pressure and force applied directly to the abdomen when snapping the pouch onto the barrier, which is especially important for patients with a sensitive peristomal area, abdominal pain, or limited dexterity.

How do I choose the right flange size?

The flange size must match the New Image pouch you are using — match the color code on the barrier box to the color code on the pouch box. Green (44mm) suits smaller frames and most average stomas; Red (57mm) and Blue (70mm) provide more peristomal coverage for larger body profiles, wider stomas, or cases where extra adhesion surface is needed. The stoma opening you cut within each barrier is separate from the flange size selection.

Can this barrier be used for a urostomy?

Yes. The Hollister New Image Flextend barrier is explicitly suitable for both ileostomy and urostomy. Urine is less erosive than ileostomy output but consistent moisture exposure still benefits from the extended wear Flextend formulation, particularly for patients with sensitive or compromised peristomal skin.

How long does this barrier typically last?

Extended wear Flextend barriers are designed to last 5–7 days under normal conditions, and up to 7 days in many ileostomy patients. Actual wear time depends on output volume and consistency, activity level, body temperature and perspiration, and peristomal skin condition. If wear time falls below 4 days consistently, consult a WOC nurse to evaluate fit, output management, or accessory use.

Do I need to use a barrier ring or paste with this product?

Not required, but often recommended. A Hollister Adapt barrier ring or paste applied around the cut stoma opening can fill any gap between the barrier and the stoma, providing extra leak protection — particularly for irregular stoma shapes or patients prone to undermining. Note that skin protective wipes should not be used under Flextend barriers.

How do I attach the New Image pouch to this barrier?

Center the barrier over the stoma and press firmly for 60 seconds. Then align the lower edge of the pouch flange with the barrier flange and press together — a series of audible clicks confirms full engagement. Unlike some two-piece systems, the New Image coupling does not require rotating or snapping in one motion; work around the flange until no more clicks are heard.

They prevent leaks, and fit great!
We have always used Huggies diapers for our babies and young toddlers because they were the only ones that never leaked or caused a rash. My second daughter was fully potty trained at 2.5 years old and never needed pull-ups. However, our third daughter, who also trained at 2.5 years old, digressed with her nighttime potty training due to instability - we moved twice within the first 6 weeks after she potty trained.
