COURSE OVERVIEW:

Welcome to the course, Address the Needs of People with Chronic Disease. Addressing the needs of people with chronic diseases requires a comprehensive understanding of the broad context surrounding chronic illness and the effective management strategies to enhance their quality of life. In this course, we delve into various aspects of chronic disease management, equipping participants with the knowledge and skills necessary to provide holistic support to individuals dealing with chronic health conditions.

Section A of this course sets the foundation by exploring the broader context of chronic disease. We begin by examining the World Health Organisation's definition and classification of chronic diseases, emphasising the significant impact they have on individuals' lives and the healthcare system.

Participants will also learn about the Chronic Care Model (CCM) and Wagner's Chronic Care Model, which provide frameworks for organising and delivering care to individuals with chronic conditions. Additionally, the Four Ps Model will be discussed, focusing on the components of chronic disease management: prediction, prevention, personalisation, and participation.

Understanding the continuum of chronic disease is essential, as it encompasses various stages and complexities that influence individuals' health and well-being. We will explore the diverse life variables that impact chronic disease management, recognising the importance of addressing social, cultural, and environmental factors in providing effective care.

Moreover, this section highlights the impacts of chronic disease on individuals' physical, emotional, and social well-being, underscoring the need for tailored strategies to address their unique needs. Participants will gain insights into evidence-based approaches for preventing and managing chronic diseases, including lifestyle modifications, treatment options, and client-focused practices.

Communication plays a vital role in chronic disease management, and participants will learn how to effectively engage with individuals with chronic conditions, fostering open dialogue and shared decision-making. Furthermore, we will examine the roles and responsibilities of various stakeholders in coordinating services and supporting individuals with chronic diseases.

Section B focuses on practical skills and strategies for addressing the needs of individuals with chronic diseases. Participants will learn how to assess the impact of chronic conditions on individuals' health and well-being, identify appropriate support interventions, and facilitate self-management strategies. Emphasis will be placed on involving individuals in decision-making processes, promoting autonomy, and addressing unmet needs through referrals and collaboration with other healthcare professionals.

Furthermore, participants will apply their knowledge and skills to determine and address the specific needs of individuals with different types of chronic diseases and lifestyle situations. Through case studies and scenario-based learning, participants will gain practical experience in developing comprehensive care plans and adapting support strategies to meet the diverse needs of individuals with chronic illnesses.

By the end of this course, participants will be equipped with the necessary competencies to provide holistic and person-centred care to individuals with chronic diseases, empowering them to lead fulfilling and meaningful lives despite their health challenges.

This course is designed to align with the content outlined in the Nationally Recognised Unit of Competency CHCCCS001.

·       Maintenance of Positive Life Roles

·       Stages of change, to establish readiness for attitudinal and behavioural change

·       Ways to communicate appropriately and effectively with a person with a chronic disease

·       Roles and responsibilities of different people in chronic disease management and their contribution to coordinated service delivery

Section B: Performance Tasks

·       Identify the person’s chronic disease conditions and seek information about its possible impacts on health, well being and ability to achieve maximum performance in everyday situations

·       Determine current treatments, and other inputs and supports by seeking information from the person or other health professionals

·       Determine the level and type of support and assistance required by the person within scope of own role

·       Adjust services and support to meet the specific nature of the chronic disease

·       Actively involve the person in the development of strategies to self manage their chronic disease

·       Discuss with the person, their family and other carers, the full range of issues that could have an impact on their well being

·       Respond to the range of issues in an integrated way

·       Support the person to understand their chronic disease condition

·       Facilitate the person to self-manage through provision of advice and resources

·       Identify and discuss any unmet needs and requirements and make appropriate referral

·       Maintain awareness of current contribution of other agencies and professionals in supporting the person

·       Identify the level and type of contribution made by family and other carers and the effects it has on the person

·       Identify and meet communication and reporting responsibilities within the support system and own role

·       Respond to variations in the person’s needs in the context of a coordinated service approach

·       Determine and address the needs of at least 3 people presenting with different types of chronic disease and lifestyle situations

No problems after a year and a half

Have been regularly buying these for my corgi for about a year and a half. The 2 pack lasts about a month or 2 each time. Pieces break off in small enough pieces that they safely pass through his digestive system. He gnaws each one down to about 1/4 the original size before we take it away and give him a new one. He continues to have regular bowel movements and has a healthy appetite so I can’t imagine any problems arising after so long. I have tried other alternatives and every other similar chew breaks off in larger pieces and didn’t feel comfortable letting him chew/eat them. I noticed there are a few 1 star reviews saying their dogs got sick off these but after a year and a half I have had 0 issues with this product. I guess it depends on the dog and how big of pieces they are swallowing. My dog ingests pieces about the size of a grain of rice so just pay attention and you should be fine.
